NSF | NSF Forms and Templates.xlsx | NSF is implementing a new pilot that will: 1) standardize collaborator and other affiliation information across the Foundation; and 2) ensure that such information is submitted in a searchable format. Effective April 24, 2017, NSF will require the use of the spreadsheet linked below as a template for identifying collaborators and other affiliations. The FastLane Collaborators and Other Affiliations screens will link to the new template and instructions on the Policy Office webpage at: https://www.nsf.gov/bfa/dias/policy/coa.jsp. The spreadsheet has been developed to be fillable, however, the content and format requirements must not be altered by the user. This spreadsheet can be saved in .xlsx or .xls format, and directly uploaded into FastLane as a Collaborators and Other Affiliations Single Copy Document. It is therefore imperative that this document be uploaded in .xlsx or .xls only. Uploading a document in any format other than .xlsx or .xls may delay the timely processing and review of your proposal
Forms and Templates for Contracts and Agreements | Contract for Professional Services Template (PDF) | The Contract for Professional Services template is used when Appalachian contracts with an institution (for goods/services provided within that organization's normal business operations and which are provided to many different purchasers) or with an individual (i.e., consultant) for services provided on a sponsored project. Click here for more information on consultants. | 6/17/2015 |
Interdepartmental Services Agreement Form (PDF) | Use the Inter-departmental Service Agreement form when one University department wants to purchase services from another department. | 3/09/2011
Subrecipient Agreement Template (PDF) | The Subrecipient Agreement template is used when Appalachian contracts with another institution under a federal grant, award, or cooperative agreement (i.e., Appalachian is giving part of its funds to another institution for a portion of a sponsored project). Click here for more information on subcontracts. | 6/19/2015
Subrecipient Commitment Form (PDF) | All subrecipients should complete the Subrecipient Commitment Form prior to being included on a grant or contract proposal with Appalachian. The form verifies information about the subrecipient in the proposal; determines if the other organization's role is that of a true subrecipient; provides a checklist of documents and certifications required by sponsors and an area for the authorized official to sign; and allows the process of establishing a subagreement to move forward more quickly at the award stage. | 1/22/2015
