NIH Template (National Institutes of Health).xlsxThe NIH template should be used for all NIH projects. Interdepartmental Services Agreement Form (PDF)

Use the Inter-departmental Service Agreement form when one University department wants to purchase services from another department.

Subrecipient Agreement Template (PDF)The Subrecipient Agreement template is used when Appalachian contracts with another institution under a federal grant, award, or cooperative agreement (i.e., Appalachian is giving part of its funds to another institution for a portion of a sponsored project). Subrecipient Commitment Form (PDF) 

All subrecipients should complete the Subrecipient Commitment Form prior to being included on a grant or contract proposal with Appalachian. The form verifies information about the subrecipient in the proposal; determines if the other organization's role is that of a true subrecipient; provides a checklist of documents and certifications required by sponsors and an area for the authorized official to sign; and allows the process of establishing a subagreement to move forward more quickly at the award stage.
